Application Procedures for SPDH Vouchers
How to Apply
- Download the application form. You need “Adobe Acrobat Reader”, to open the form. If you do not have it, you can download it from the Adobe Homepage. See the Related Links section. The application is form fillable but cannot be saved.
- Print out the completed application form.
- Put your signature on the last page.
- Submit, mail, or fax your completed application form to the SPDH Program office at ‘Iliahi 201 by the deadline for PRIORITY processing.
ONLY FULLY COMPLETED APPLICATIONS WILL BE CONSIDERED
If you are not yet a member of the SPDH program, you need to contact the program office to complete an intake appointment to determine if you are eligible for services.

Applications received PRIOR to the start date will not be considered.
Applications must be postmarked by the end date for priority processing however late applications may be considered if funds are available.
